Dentists: You Can’t Create Enough Happy Smiles to Outpace Faulty Business Systems

A Harsh Truth
Dentists: let’s take a moment to reflect on a harsh truth—one that we don’t often like to face.
A business that its customers love can still fail.
Think about that for a moment. Let it sink in.
A business that inspires loyalty, that fills a community with joy, that has people raving and singing its praises—can still close its doors forever.
I was reminded of this painful reality recently when two small businesses on Main Street, the same street as my dental practice, shut down for good. One was a coffee shop that brewed the kind of coffee people would drive miles to sip, a cozy refuge where people would linger for hours. The other was an art studio that buzzed with creativity, a space where people found expression, connection, and inspiration. Both of these businesses were beloved. Both had devoted followings. Both are now gone.
If love from customers were all it took to succeed, those businesses would still be thriving. But love, as it turns out, isn’t enough.
The tragic irony is that a business-owner can make their customers smile, without making a great living.

Why Making Them Smile Isn’t Enough
Many of us—dentists, in particular—get into business because we’re driven by the desire to serve. We want to make a difference in people’s lives. In our case, we want to create confident, healthy smiles. That’s a beautiful purpose, and it’s essential to our success. But it’s not self-sufficient for long-term survival.
You see, a business is like a bridge. The beauty of the design may inspire awe, but if the structural beams are weak, it will collapse under its own weight. Without solid financial fundamentals and strong business systems, even the most beloved businesses crumble.
As dentists, we’re in a unique position. Most of us don’t think of ourselves as “businesspeople.” We think of ourselves as healthcare providers, artists, and problem-solvers.
But we cannot afford to ignore the business side of what we do. Why? Because our purpose—to transform lives with healthier, happier smiles—depends on it.
In dentistry, we love to focus on patient care. And yes, happy patients who trust you and rave about your practice are key. But here’s the painful truth: you can’t create enough happy smiles to outpace faulty business systems.
Learning From Main Street
Let’s go back to those two businesses that closed on Main Street. I’ve spent some time thinking about what went wrong, and while I don’t have access to their financials, I’d wager that their failures were not about the product or the experience they provided.
The coffee shop served exceptional coffee; the art studio inspired creativity. The problem likely lay in the foundation of their businesses—their financial models, pricing strategies, and systems for growth.
Maybe their rent became unsustainable.
Maybe they didn’t adapt to shifting customer behavior.
Maybe they didn’t plan for the slower months, or they failed to scale their operations properly.
Whatever the reasons, they weren’t rooted in a lack of customer love.
Here’s the takeaway for us as dentists: It’s not enough to have happy patients who love what you do. You need systems and strategies that ensure your practice is profitable, sustainable, and scalable.
Building A Business That Lasts
So, how do we avoid the fate of the coffee shop and the art studio? How do we ensure that our practices aren’t just loved but built to last?
- Know Your Numbers
It starts here. Understand your revenue, expenses, profit margins, and cash flow. Measure everything, from your case acceptance rate to your patient retention rate. Numbers don’t lie, and they will tell you where your practice is thriving and where it’s bleeding. - Systematize Everything
Patients love personalized care, but behind the scenes, your business needs to run like a machine. Scheduling, billing, follow-ups, patient education—every process in your practice should be repeatable, efficient, and scalable. - Price Properly
The market determines value. Too many of us undervalue our services. We’re afraid to raise prices because we think patients will leave. But if your practice isn’t profitable, you’re not serving anyone—not your patients, not your team, and certainly not yourself. Others over price their services. You may think your crowns should be priced at $2,000 each….but your local market might be telling you otherwise. - Adapt to Change
The businesses on Main Street didn’t fail because they weren’t loved; they likely failed because they didn’t adapt. The marketplace is always shifting. Trends, technologies, and customer expectations evolve. As practice owners, we must stay ahead of the curve. - Invest in Leadership
As the owner of your practice, you are its CEO. That doesn’t mean you need an MBA, but it does mean you need to embrace your role as a leader. Surround yourself with mentors, take courses on business management, and always be learning.
Purpose + Profit
I want to leave you with this: Our purpose as dentists is to care for people, to help them live healthier, more confident lives. That’s why we do what we do. But purpose without profit is like a car without fuel. You won’t get far.
The businesses that succeed—the ones that survive and thrive—are those that combine purpose with sound business fundamentals. They create value for their customers while ensuring they’re financially secure.
Don’t let the fate of the coffee shop and the art studio be your fate. Build a practice that your patients love, yes. But also build a practice with a foundation so strong that it can weather any storm.
Because a business that transforms lives, a business that creates happy smiles, deserves to be here for the long haul. And that’s only possible if the bridge between passion and profit is solid.
It’s not enough to make people love you. You have to build something that lasts.
